Introduction to How to Get a Paid Family Caregiver with CDPAP

Joanne Bailey was a caring mother and grandmother who had worked hard her whole life to care for her family. The matriarch of the Bailey family, Joanne’s life had always been about giving. Now in her late-70s, she suffered from a number of health complications and physical disabilites that caused her to need help with a number activities of daily living. She also required wound care as a result of a recent surgery.

Molly, her loving daughter, was there as often as possible. However, being both a mother and wife, she was also juggling her own work and family responsibilities – not leaving her nearly enough time to properly address her mother’s needs.

Molly wanted to get her mom a home attendant from a local agency, but she knew that her mother would be uncomfortable having a stranger in her house. Also, even if she could get her mother to accept a home home health aide (HHA), the HHA wouldn’t be allowed to administer the wound care her mom needed.

Then a social worker told Molly about the Consumer Directed Personal Assistance Program (CDPAP), a New York State government sponsored program that lets home care consumers hire their caregiver of choice – including most family members.

Through CDPAP, Molly was able to get paid to take care of her mom. Since CDPAP caregivers are allowed to administer skilled services, Molly was able to help her mom with the would care she required. CDPAP enabled Molly to give her mom the kind of care that only a daughter can provide, without compromising on her responsibilities to her own family

Eligibility to Get a Paid Family Caregiver with CDPAP

Am I eligible to Get a Paid Family Caregiver with CDPAP?

The following three factors will give you an idea if you may be eligible:

  1. You must have Medicaid (we can help you apply if you do not yet have Medicaid).
  2. You must require home care.
  3. Your must be able to direct your own care or have a representative that can direct it.

Simply meeting the above criteria does not guarantee that you are eligible, but we recommend that you contact us to see if you can enroll.

How to Apply

Where can I get an application?

If you want to apply to get a paid family caregiver and you’re trying to figure out where to start, please know that there are often several steps involved in the application process, and it is not just a matter of filling out a single form. We break down the process into three components:

  1. Assessment(s) – Most applicants must be assessed by a nurse (sometimes more than once) before enrolling. The assessment(s) can be done in the comfort of your home and can often be completed remotely.
  2. Forms – Certain forms are required. We will help you with whatever you need.
  3. Select your caregiver of choice. Most family members can get paid to care for you. However, it doesn’t have to be a family member. You can hire a neighbor, friend, previously hired aide, and almost anyone allowed to work in the United States.

It really depends on your situation. However, you will certainly need something. We recommend you give us a call so we can give you a clear picture of what you will need. We will even help you with gathering the necessary forms from your doctor and other providers.

Hours of Care

How many hours of home care will my family member get paid to provide?

Not all consumers receive the same number of hours. In fact, there are often vast differences between consumers in the number of hours for which they are approved. Usually, your Medicaid Managed Care plan will make the determination on how many hours you will receive.

Allowed Family Caregivers

Which family members can get paid to provide care under the CDPAP program?

Consumers can hire most family members. However, there are some exceptions.

The following are answers to some common questions that come up regarding which family members can be hired as paid family caregivers under the CDPAP:

Can children get paid to take care of their parents?

Yes. Children can get paid to care for their parents.

Can parents get paid to take care of their children?

It depends. Parents can get paid to care for their children that are age 21 or older. Parents cannot get paid to care for children younger than 21. Please see here and here.

Can siblings get paid to take care of each other?

Yes. Siblings can get paid to take care of each other.

Can spouses get paid to take care of each other?

No. Spouses cannot get paid to care for each other. Please see here.

Living in the Same Home as Your Family Caregiver

Can a family caregiver live at the same address as the consumer under CDPAP?

It depends on the situation. The government’s policy states as follows: “a consumer directed personal assistant may include any other adult relative of the consumer who does not reside with the consumer or any other adult relative who resides with the consumer because the amount of care the consumer requires makes such relative’s presence necessary”.

Are family caregivers required to have any license or certification under CDPAP?

Okay, we gave it away. Caregivers, including family members, are not required to have any license or certification under CDPAP.

Who trains the family caregiver?

The consumer or their representative trains their own caregiver – including family members.

Family Caregiver Requirements for CDPAP

What required documentation must family caregivers provide under CDPAP?

The same documentation is required for all CDPAP caregivers – whether they are family members or not.

The following is a list of the required documentation for your family caregiver at our agency:

  • UNEXPIRED picture ID such as NYS ID, Driver’s License, Passport, Permanent Resident Card or Work Authorization Card. You might need a second form of identification. We only take original IDs; copies will not be accepted.
  • Social Security Card
  • Pre-employment Physical (Please see Form below) – No later than 6 months of application date
  • Rubella Lab Report – The results must show Immune or high (Lab Report). If results in the lab report are low, proof of 1 vaccine must have date of administration and name and license # of the person who administered the vaccine.
  • Rubeola (Measles) Lab Report – The results must show Immune or high (Lab Report). If results in the lab report are low, proof of 2 vaccines must have date of administration and name and license # of the person who administered the vaccines.
  • Two step TST (PPD) or IGRA QuantiFERON within previous 12 months and TB Risk Assessment. *For Positive PPD: You MUST have a CHEST X-RAY Report (NOT a Doctor’s note) which contains the Doctor’s signature and license #.

Types of Home Care Services Allowed

What types of home care services can Family Caregivers provide under CDPAP?

The types of care that caregivers can provide under CDPAP are much broader than those that traditional agency provided home health aides (HHAs) and personal care aides (PCAs) are allowed to administer. This goes for family caregivers or any other chosen CDPAP caregiver.

Unlike home health aides and personal care aides that are only allowed to provide custodial care such as dressing, bathing, and meal preparation, CDPAP caregivers are also allowed to provide skilled care such as wound care, giving insulin shots, and suctioning tracheostomies. Please see here and here. This is potentially a very significant benefit for home care consumers that require ongoing skilled care.

Paid Family Caregivers and Medicare

Does Medicare cover a paid family caregiver via CDPAP?

Medicare does not cover CDPAP. CDPAP is a Medicaid based program, and you must have Medicaid to enroll. Many consumers have both Medicare and Medicaid. Those consumers can enroll because their Medicaid will provide coverage.

If you only have Medicare and do not have Medicaid, we may still be able to help. We have Medicaid specialists that can guide you through the process of applying for Medicaid.

What if I don’t have Medicaid?

CDPAP is a Medicaid based program, and you must have Medicaid to enroll. But don’t be discouraged if you don’t have Medicaid. We have Medicaid specialists that can guide you through the process of applying for Medicaid.

Advantages of a Family Caregiver with CDPAP VS Traditional Home Care

Are there any advantages that a family caregiver under CDPAP affords as opposed to traditional home care?

The NY State government covers the costs of home care for consumers that are both financially and clinically eligible. This government sponsored home care can come in different forms. Traditional home care is provided by a home care agency via home health aides or personal care aides, while CDPAP home care allows the consumer to hire their caregiver of choice – including most family members.

The following are some advantages of a family caregiver via CDPAP VS traditional home care:

  • Consumers are often more comfortable receiving care from their relatives.
  • Family Caregivers are not required to have special certification or licensing under CDPAP.
  • Family Caregivers can administer skilled services such as wound care, giving insulin shots, and suctioning tracheostomies under CDPAP.
